Two service lines, knee pain and vein treatment, a 2024 ad attempt that had stopped at $16 a lead, and a front desk that could not chase leads the same day.
The situation
The practice had no way to tell an ad-driven enquiry from a referral, and the standard Meta pixel is restricted for medical advertisers, so any campaign was optimising on nothing.

What we built
- Two screener lead campaigns, one per service line, each fronted by a short symptom questionnaire inside the Meta lead form: pain location, duration, insurance.
- Pixel and Conversions API rebuilt for a health advertiser, with a custom “lead to scheduled” event so booked screenings feed optimisation, not just form fills.
- Every lead into a GoHighLevel pipeline with SMS and email follow-up inside minutes and appointment booking.

How to read it
Qualified form submissions, not patients or appointments. The October and November rows are shown on purpose: every medical account on our book converts at $3–4 for about two months, then cost per lead roughly doubles as the local audience saturates. The answer is a creative refresh at month three, planned in advance.
